WestMass ElderCare awards grants to agencies through the federal Older Americans Act to fund a number of priority services for elders, such as: legal assistance, transportation, home repair, community-based services, in-home services, caregiver services including respite, and a variety of health programs.  Services target individuals of greatest economic and social need, including minority and LGBT populations.

Pioneer Valley Aging and Disabilities Resource Consortium (PVADRC)
WestMass ElderCare, Inc. is a member of the Pioneer Valley Aging and Disabilities Resource Consortium, a collaborative effort of the Aging Services Access Points in Western Massachusetts and Stavros Center for Independent Living.  The PVADRC serves as a single point of entry into the long-term supports and services system for older adults and people with disabilities. Sometimes referred to as a “one-stop shop” or “no wrong door” system, the PVADRC’s goal is to eliminate many of the frustrations consumers and their families experience when trying to find needed information, services, and supports.

Councils on Aging
WestMass ElderCare partners with the local Councils on Aging in planning and coordinating services for older adults in the region.

Councils on Aging Senior Centers are designated as Community Focal Points within their communities, providing older adults with access to information, services and opportunities.

WestMass ElderCare partners with local Senior Care Options Programs to provide the expertise of our Geriatric Support Services Coordinators (GSSC) in assisting members maintain independence in the community through a variety of services.

Senior Care Options is a comprehensive health plan that covers all of the services that can be paid for under Medicare and MassHealth. Members who join SCO will have a primary care physician who is connected with the Senior Care Options Program. The physician and a team of nurses, specialists, and the WestMass ElderCare work with the individual to develop a plan of care to address the needs of the individual. Members enrolled in SCO have 24-hour access to the team, support services, and have active involvement in decisions about their health care.

One Care
Each One Care member is offered the expertise of WestMass ElderCare’s Long Term Services and Supports Coordinator, in planning their long-term community-based supports.

One Care is an option for people with disabilities to receive services provided by both MassHealth and Medicare. With One Care, there is one plan, one card, and one person to coordinate your care with the support of a multi-disciplinary medical team.

WestMass ElderCare partners with local PACE Programs to provide supplemental services such as home delivered meals and Adult Foster Care.

The Program of All-inclusive Care for the Elderly (PACE) is administered by MassHealth and Medicare to provide a wide range of medical, social, recreational, and wellness services to eligible participants. The goal of PACE is to allow participants to live safely in their homes instead of in nursing homes.
