My Life, My Health

This free program provides information and teaches practical skills to help you or your loved one live better with chronic health conditions such as diabetes, arthritis, and hypertension. By participating, you will learn valuable tools and gain motivation needed to manage the challenges of living with these conditions. The goal of this course is to enable participants to build self-confidence and assume an active role in maintaining their health and living well in spite of their challenges.
My Life, My Health is an evidence-based chronic disease self-management program developed by Stanford University and offered in communities throughout the United States and several other countries. This program has many benefits for adults experiencing chronic health conditions such as hypertension, arthritis, heart disease, stroke, lung disease, and diabetes. Family members, friends and caregivers can also benefit by participating.
