Home Care services are tailored to meet individual needs of eligible elders 60+, and are coordinated through provider agencies.  These services may include individual caremanagement, personal care assistance, meal preparation, laundry, housekeeping, companions, respite for caregivers and more. 

Elder Nutrition program provides well-balanced, hot noon meals for elders 60+ (and their spouses), Monday-Friday, at a congregate mealsite or delivered to their homes.  Congregate mealsites provide opportunities for social interaction, nutrition education and volunteer activities, and are located within the community.  Hot meals are delivered to frail, homebound elders by trained drivers, who may help to communicate the elder’s need for additional assistance. 

Money Management program is an inhome service that assists low income, frail elders who have difficulty writing checks, balancing checkbooks and managing household finances. 

Adult Family Care provides a foster home for older adults, persons with physical and mental disabilities, and AIDS.  AFC provides professional nursing and social work support.  Caregivers and participants are matched on the basis of common interest, location, and the specific needs of the participant. 

Group Adult Foster Care is a Medicaid program that provides daily personal care services to adults with impairments, who live in managed housing complexes.

Personal Care Management, provided through MassHealth's PCA Program, provides assessment, evaluation and skills training for eligible Consumers who wish to self-direct their own care, as the employer of their personal care attendants.

Coordination of Care, Nursing Home Screenings - nurses assess the elder’s health and needs to determine most appropriate plan for care, and explore community options to nursing home placement, working closely with family.  Elders are also screened for nursing home placement for short term and a long term stays; also pre-screening to determine eligibility for Medicaid coverage of nursing home placement.

Long Term Care Ombudsman program provides a way for nursing and rest home residents to have their complaints addressed; and also provides information about nursing and rest homes.  Trained volunteer Ombudsmen, certified by the Executive Office of Elder Affairs, visit nursing and rest homes on a regular basis.

Speaker's Bureau - an experienced, resourceful, professional staff member will share valuable information with your organization.  A resource for community organizations and businesses with an interest in learning more about elder services.
