REGISTERED NURSE  -  ADULT FAMILY CARE- Full Time-35 hours. Tired of the stress but still want to use your nursing skills in meaningful work? No weekends or holidays! Reasonable caseload. Energetic, well-organized person to join interdisciplinary team to assess and monitor adults with disabilities being cared for/living in private homes.  CASE MANAGER - Responsible for coordination of income eligible supportive services to individuals 60+ years to maintain maximum independence in a home environment; assess needs and eligibility, develop service plans, and ongoing case management. Assessment and documentation skills a must. Minimum requirements must be met. Minorities, individuals fluent in Spanish, and older persons are encouraged to apply. A minimum of a Bachelor’s degree in social services or related area, valid driver’s license, reliable transportation, and computer skills required.  Annual salary in the thirty’s.  Regular hours no weekends or nights. Bachelor degree in social service or related area is also essential. Social work license desirable.
