Our Information and Referral Department is staffed with friendly, experienced, trained Specialists ready to help you with your questions and to provide you with answers. If we do not have the answer right away, we know where to find the answer. We will help you find the services you need to maintain independence at home. For those who are currently in a nursing, hospital or rehabilitation facility and wish to return home, we can assist you transition to community services.
